Referrals

Community Health Matters

Healthy Valleys provide a range of activities, some of which are drop in, open to all but require booking or for a specific group of people.

For intensive 1:1 support, you need to complete a referral form, which you can do below.


If you would like to self-refer for support, please complete the form below and one of our team members will be in contact with you as soon as we can.
Essential referral criteria (Please tick to confirm)
Resident in Clydesdale or part of rural South Lanarkshire
Expecting a baby/caring for a child under 5 years old
Lonely and/or isolated
Aged between 18 and 50
Mild to moderate mental health condition
Aged 50 or over
Please note that as a service we are currently unable to offer support to individuals who have severe mental health conditions including Dementia, are chaotic because of substance misuse, or who need direct support with personal care or who require significant support with mobility.

Experiencing food poverty
